A functional capacity assessment report is a broad and in-depth assessment of a person’s ability to participate in everyday life activities. They identify the support needs of the person, identify opportunities for capacity building, make recommendations on supports required and cover all areas of function.
Functional capacity reports are commonly asked for by the NDIS. They are often requested if:
-your circumstances have changed
-you are asking for a review of your funding
-you feel that there is a mismatch between your needs and your funding
At Connecting Dots 4 Kids our FCAs are broad-ranging and will include:
- Indepth interview with parents/carers
- Functional activity assessment
- Reading and collaborating reports and information from key supports for your child
- Formal assessments as required by NDIS to show functional needs - i.e. Vinelands, Pedi-Cat and others as required.
- Recommendations to meet your child's current support needs
At Connecting Dots 4 Kids we offer OT FCAs to children up to 12 years of age or older by negotiation. For children located in the Gawler Area or Yorke Peninsular, these assessments can occur face-to-face but we can also offer this service via telehealth anywhere in the state.
