Your child's information and privacy is extremely important at Connecting Dots 4 Kids. Your child’s personal information, including information about others who are key stakeholders in their life, is gathered as part of the assessment and treatment process and is kept securely. Connecting Dots 4 Kids utilizes an electronic database (Cliniko) which meets HIPAA requirements for storage of secure health information.

At Connecting Dots 4 Kids, we may work closely with other agencies to coordinate the best support for you, your child and others. This means your informed consent to share information about you and your child will be sought and respected in all situations unless:

1) disclosure is authorized or required by law, or

2) (a) it is unreasonable or impractical to seek consent; or consent has been refused; and

(b) the disclosure is reasonably necessary to prevent or lessen a serious threat to life, health or safety of a person or group of people.

Your child’s personal information is retained in order to document what happens during sessions, and enables your therapist to provide a relevant and informed service to you and your child.

 

At Connecting Dots 4 Kids, we value your child's privacy in the highest regard. However, from time to time, we do use electronic media to provide therapy services, i.e. telehealth services, email exchanges to plan details for upcoming therapy appointments, videoing for coaching, photographs to make visuals, etc. We will not use your child's image for advertising without your prior written agreement. We do use email as a way of communicating basic details like appointment changes, contact details and sharing documents for printing prior to telehealth appointments and to communicate with other service providers involved in your child's care. We may also share reports containing confidential information about your child's presentation and diagnosis via email from time to time. Please provide us with written correspondence if you do not wish for any information about your child to be shared via email.

You may request access at any stage to your child’s personal information kept on file, subject to exceptions in the relevant legislation. Please discuss with your child's therapist which information you are needing and they will discuss with you how this information can be provided.

Feedback enables us to optimize your therapy journey and we may ask you periodically, either verbally or in writing for feedback.
